Engage in direct work with clients through the Community Resource Navigator Program and develop a better understanding of how social determinants of health affect peoples' lives. Partner with clients to identify social and legal services, assist them in connecting to needed resources, follow up to be sure that the connection was made and evaluate the quality of the resources provided.
